Baptist Sports Medicine will offer physical examinations to Lipscomb students for only $10.00 on Friday, May 1 at 5:00 p.m., in the high school Auxiliary Gym. Coaches and parents: in order to expedite the screenings, please print the following physical examination packet, complete the sections marked “Completed By Patients,”  and bring it with your student athletes on May 1st.  If you would like to have the vital section filled out ahead of time, please see Ken in his office on Mondays and Wednesdays between 10:30 am and 12 noon.  (Note: you will still need to see the physicians on May 1st to complete the physical).
  These physicals meet the requirements for all TSSAA athletics.
